Please find the materials and documentation in support of my request for a competency-based portfolio assessment enclosed. I am petitioning for the maximum amount of allowable credits in my program: 9 Lower Level and 15 Upper Level PLAR Applied Studies credits. The subject areas of my learning include:

The evidence supporting my request includes samples of my professional and volunteer work experience. My experience includes the development, application, analysis and evaluation of various projects. These projects include the creation of a staff training manual, program content and multiple events.
I look forward to discussing these areas of learning further at the assessment interview upon completion.
